HIPAA 5010 Implementation for Medical Billing

HIPAA 5010 was adopted to replace the current version of the standard that covered entities must use when conducting electronic transactions. Version 4010 is currently being used under HIPAA standards.

Although HIPAA version 5010 gets much less attention than ICD-10 medical codes,it is just as important and physicians, medical practices and other health providers, should already be working with vendors on the version 5010 implementation.

Testing with external trading partners began in January of 2011. It is very important that you test as early as you can and often.

Here are some important 5010 compliance testing dates to be aware of:

  • January 1, 2011 Level I compliance-ability to process 5010 transactions for testing and transition with able trading partners
  • January 1, 2012 Level II compliance-all covered entities must begin using 5010 transactions

Simply put, transactions are electronics exchanges involving the transfer of health care information between two parties for specific purposes, such as a health care provider submitting medical claims to a health plan for payment. The Health Insurance Portability & Accountability Act of 1996 (HIPAA) named certain types of organizations as covered entities, including health plans, health care clearinghouses and certain health care providers. HIPAA adopted certain standard transactions for Electronic Data Interchange (EDI) for the transmission of health care data, as well. These transactions include, but not limited to:

  • claims and encounter information
  • payment and remittance advice
  • claims status
  • eligibility
  • enrollment and disenrollment
  • referrals and authorizations

Unlike the current 4010 transaction standard, version 5010 is more specific in the type of data it collects and transmits over the course of a transaction. 5010 also has clear situational rules built in which will help enhance the understanding of claim corrections, reversals, recoupment of payments and the processing of refunds.

For example, HIPAA 5010 will increase the diagnostic field size to accommodate the increased size of ICD-10 codes. Some other changes include:

  • a version indicator that distinguishes between ICD-9 and ICD-10 codes
  • format changes that will increase the number of diagnosis codes allowed on a claim

Interestingly, the 5010 format does not require the use of ICD-10 codes. However, it will be able to recognize and distinguish between the ICD-9 and ICD-10 medical code sets, which may help in a future with dilemmas of billing utilizing the dual code sets.

All health providers have to establish steps for preparing for 5010 Implementation and it might include the following:

  • Software must be modified to produce and exchange the new formats (e.g. trading partners must be able to read incoming 277CA transactions).
  • Review business processes to ensure changes are not necessary to capture additional data elements not previously required (e.g. Impact of patient registration, billing, and claim reconciliation).
  • Contact your vendor and/or clearinghouse to ensure products and processes are updated (e.g. license includes regulation updates, and will the upgrade include acknowledgement transactions 277A & 999).
  • Trading Partners should contact their local Medicare-Fee-For-Service contractor (MAC) for specific testing schedules.

Before I discuss some Graves’ Disease diet tips that can help restore your health back to normal, let me first tell you that not only am I a licensed healthcare professional, but I was personally diagnosed with Graves’ Disease. And nutrition definitely played a huge role in restoring my health back to normal. I will warn you that being a holistic doctor I am biased towards natural treatment methods, so while most endocrinologists label Graves” Disease as “incurable”, based on my own personal experience I believe that Graves” Disease can be cured, and one’s diet is important in restoring their health.

So if you are looking for Graves’ Disease diet tips that will help restore your health back to normal, then you are going to enjoy reading the following information. As I just mentioned, diet can play a big role in treating Graves” Disease naturally. However, I will tell you that while eating healthy and incorporating nutritional supplements can be beneficial, there are other factors that will also play a role in your recovery, which I will briefly mention in this article.

I also will warn you that different doctors will have varying opinions as to what is considered to be “healthy” with regards to one’s diet. And let’s be honest for a moment…nobody eats the “perfect” diet, as in my opinion it’s okay to be “bad” every now and then. For example, I consulted with a patient once who loved milkshakes, which as you might guess isn’t too healthy. And while it would be ideal for this person to completely eliminate milkshakes from her diet, having one every so often usually isn’t a big deal. And the same goes with other foods.

Of course there are exceptions. For example, someone with celiac disease probably won’t be able to eat a gluten-based food “every now and then” without having a bad reaction. Another example involves someone who can’t “settle” for a milkshake every now and then, or an occasional slice of pizza, etc. In other words, some are better off staying away completely from the “bad” foods, or else they begin consuming too much of it.

Testing For Food Allergies Is A Good Idea

Anyway, one of the first things I will recommend before anyone changes their eating habits is to get tested for food allergies. Going to a regular allergy doctor might not be the best option, as usually they will test for only IgE antibodies. You might want to visit a holistic doctor and receive an ELISA/EIA panel, which also measures the IgG antibodies, and is usually more accurate (although this does depend on the lab that performs the test).

Another alternative is to consider a GI Health Panel. The company Diagnos-Techs offers this type of test, which measures the health of the gastrointestinal tract and includes over one dozen tests that utilize saliva and stool specimens. They also offer a FIP-Food Intolerance Panel, that will determine whether you are allergic to four of the most common foods (gluten, soy, milk, and egg proteins).

A less costly method is to conduct an elimination diet, where you eliminate all of the common food allergens (wheat, soy, dairy, etc.), and then slowly introduce a potential allergen, one at a time. There are different ways of doing this, but here is an example. You can go on a purification program (described shortly), and then once the purification program is completed, introduce wheat for 3 days, and see how your body reacts. Just make sure you don’t add more than one potential allergen, as if you were to add both wheat and dairy for example, and had a bad reaction, you wouldn’t know which allergen was responsible.

A Purification Program Can Be Beneficial

As I just mentioned, many people can benefit from a purification program. There are different types of these programs, and when doing one it is wise to be under the guidance of a competent healthcare professional. When I put one of my patients on a purification program, I prefer having them avoid gluten, dairy, soy, and other common allergens for at least twenty one days. Once again, the reason for this is because they are common allergens.

On the other hand, I encourage them to eat plenty of vegetables, some fruits, as well as chicken, turkey, and certain types of fish. I do recommend that they purchase organic food, at least with regards to the meats. While it would be great if they can buy everything organic, I realize some people can’t afford this, and so if you must pick and choose, I’d recommend to eat organic meat, and try to stay away from non-organic vegetables and fruits that are high in pesticides. These aren’t necessarily the only foods I recommend, as most people can consume a small quantity of raw nuts (sunflower seeds, almonds, etc.), as well as some other foods.

An Actual Example Of A Graves” Disease Diet:

Here is an example of a daily protocol when I first went on my Graves’ Disease diet. Keep in mind that this is just an example, and is not a diet that I stayed on permanently (although it did train me to eat better, and I still eat healthy to this day):

Breakfast:

• Protein drink which contained 2 cups of purified water, I cup of mixed berries (raspberries, blueberries, and blackberries), a healthy form of protein powder, and 1 teaspoon of flax seed oil (after one month I added one raw organic egg to this).

Mid-morning snack:

• Organic apples and raw sunflower seeds

Lunch:

• Grilled chicken salad: organic mixed greens and spinach, organic grilled chicken

Mid-afternoon snack:

• Another protein drink (very filling, and actually tastes pretty good)

Dinner:

• Organic roasted turkey

• Some type of vegetable (i.e. squash)

After-Dinner Snack:

• 1 cup of raw nuts (almonds, Brazil nuts, walnuts, cashews)

Although this diet is relatively healthy, you’ll notice that it isn’t perfect. I probably should have incorporated more vegetables, but overall it was a healthy diet. Today I still eat healthy, but I do incorporate some whole grains occasionally (for example, I will have a turkey wrap or sandwich with whole wheat bread), and every now and then I’ll even go out and get some pizza, eat some chocolate, etc.

If you currently eat a lot of junk food and/or have strong sweet and carbohydrate cravings, I wouldn’t expect you to change overnight. Under such circumstances it is best to take it slow, and when I first consult with a patient that has strong sweet and carb cravings, I put them on a protocol to help them get rid of these cravings, and thus make the transition to a healthier diet much easier. I personally was brought up eating foods like Fruit Loops, Burger King Whoppers and fries, and drank plenty of Hawaiian Punch. And while I was already eating much healthier when I was diagnosed with Graves” Disease, when I first began trying to eat healthy it did take some time.

Drink Plenty Of Purified Water

You also want to drink plenty of purified water during this time, and avoid any soft drinks, and even most juices, which can be high in sugar. As for what type of water you should drink, different doctors have different opinions, but I recommend either water that has gone through a reverse osmosis process or distillation, and then is enhanced with minerals. Some will disagree with me, preferring water without minerals added. Either way, stay away from the tap water, and I’d also be cautious about drinking spring water unless if you know the source is a good quality.

What Are Goitrogens?

There are also certain goitrogens you should try your best to avoid. Goitrogens interfere with the function of the thyroid gland, which admittedly isn’t as big of a factor in someone with a hyperthyroid condition like Graves” Disease as it would be with someone with a hypothyroid condition. But still, you want to at least minimize the following foods, and some of them you will want to avoid completely. I personally ate some of the following goitrogens in moderation (i.e. spinach and broccoli), while completely avoiding others (i.e. soy). Anyway, here are some of the goitrogens to be aware of:

• Soy

• Broccoli

• Brussel sprouts

Where Should You Shop?

As for where should you shop, although you can shop at your local health food store, I personally like Trader Joes, which is a national chain that has plenty of natural and organic food at affordable prices. Some “regular” grocery stores, and even places like Wal-Mart sell some natural and organic food as well. While my wife and I love shopping at Trader Joes for most of our items, we do buy some of our foods at a local health food store, and a few items in a “regular” supermarket.

Other Factors Besides Diet Can Impact Your Health

Besides eating healthy, there are some other factors that can affect your health. Since it’s difficult to get all of the vitamins and minerals you need through your diet, taking some quality nutritional supplements can be beneficial (i.e. B vitamins, Vitamin D, iodine, essential fatty acids, a multi-mineral supplement, etc.). As I’m sure you know, it’s also beneficial to be on a regular exercise program. Another big factor for people with Graves” Disease is to manage the stress in their lives, as while just about everyone has to deal with stress, we all have different coping mechanisms. And those people that don’t do a good job of managing the stress in their lives are more likely to have stressed out adrenal glands, which can both lead to and exacerbate an autoimmune condition.

Health is Wealth – Do You Agree?

I agree whole-heartedly with the saying “Health Is Wealth”. If a person is not healthy then he or she will find it very difficult or even impossible to enjoy life, even though he or she may be very rich.

If I am asked to choose one from two of the following things, which will I prefer: health without wealth, or, wealth without health? Without hesitation I will choose the former. There is no doubt.

Wealth is generally associated with how much money a person has, that is, how rich he is. An uncle of mine is very rich. He has just about everything anyone can hope for, and some things no one dares to hope for. That is how rich he is. He lives in a huge mansion guarded by huge dobermans and sophisticated burglar alarms. He has expensive cars, furniture, paintings and other rich man’s toys. Well, he has everything except good health.

Everywhere he goes he carries with him a bag of medicine. He has to take pills everyday for his weak heart, diabetes and hypertension. He is about sixty years old and has spent his life accumulating wealth. Without doubt he has succeeded in becoming very rich. However he has also succeeded in becoming very unhealthy too. He laments about his lack of health and says that how he wished that he was young and healthy again. Unfortunately, he cannot regain his health again.

This example of wealth without health is not what I want to follow. It is ridiculous to have so much material wealth but being unable to enjoy it. What is the use of money when one has to take so much medicine everyday just to stay alive? How can one enjoy life when one’s body is weakened by disease and pain? No wonder my uncle laments so.

Health without wealth is far better. That is what I have now. I do not have to worry even a bit about what I can eat or do. I eat and do what I like because my body is healthy. Of course I do not abuse it by taking drugs or overexerting myself. I take good care of my body. In that way I stay healthy.

With health I can go about my work with joy. If I become rich, then that is fine. Then again, I may lose my riches, but that is all right too. I can always do the things I enjoy whether I am rich or not. I can play games, swim in the sea, climb hills, eat delicious fruits and appreciate the things of this beautiful Earth. This is indeed wealth – to be healthy and joyous.

Destined to Fail – Study of Brand Launch Failure in Healthcare OTC Category

Master Sun Consulting has done a study of the “Brand Launch Failures in the OTC Healthcare Category”. The OTC products in the healthcare sector can be defined as the products which help in curing any day-to-day therapeutic ailment like cough, cold, fever, headache or body ache, weakness, tiredness.

Some of the brand launches that were studied were the launch of Savlon, Disprin Plus (later Disprin Paracetamol), Chawan Junior, Duractin, Crocin Pain Relief, Crocin Syrup for Children, Dettol Plaster, Dettol Extra (almost a beauty soap), Vicks Cough Syrup, Gacidity (an antacid), Pediasure (an adult nutrition brand).

Some of the major reasons for failure observed by Jasravee Kaur Chandra, a Healthcare Branding Specialist were “Failure to understand usage of product by consumers, consumers’ attitudes and beliefs, consumer’s relationship with a brand and positioning of the brand vis-à-vis competition”.

One of the most notable failures was the Savlon launch campaign. Savlon, a Johnson & Johnson brand, could not succeed since it went against the grain of consumer beliefs about the category. It was widely held belief that for an antiseptic to be effective it has to be perceived as making an effect- the sting of Dettol. Therefore Savlon’s no-sting & sweet smell went against the basic consumer attitudes to anti-septic category. According to Sarvajeet Chandra. Managing Partner, Master Sun Consulting, “the clinical smell of Dettol enhanced the perception that the anti-septic was working. Consumers were not expecting to spray a perfume on their wounds; they wanted an anti-septic lotion which was seen to be waging a war”

The other notable example was the failure of Disprin Plus by Reckitt Benckiser. The marketers extended Disprin brand into the Paracetamol category to launch Disprin Plus. Since Aspirin comes under price control the profitability of Disprin was very low. Even today the brand is sold at less than one rupee per tablet. To circumvent this issue, company launched Disprin Plus in 2001. It only had Paracetamol as the active ingredient (instead of Aspirin). The idea was to leverage the Disprin brand equity and also maximize the profitability of the brand (as Paracetamol is out of price control).

Again we see that the marketers dug their own graves without paying much regard to customer beliefs and attitudes. Jasravee observes “The core benefit of the brand was tinkered with a view to leverage the equity of the Disprin mother brand. What was ignored was the fact that ‘ingredient’ was part of the equity. Re-positioning efforts as Disprin Paracetamol also did not work. By 2005, the writing was on the wall”. Again the results were akin to the Savlon disaster. Sarvajeet remarks light heartedly that “It was like introducing a Bacardi brand of beer. A similar mistake was initially made when Crocin was extended into pain relief to take advantage of the analgesic market”.

Similar example of marketers not doing their homework properly was the launch of Chawan Junior by Dabur. The consumers couldn’t connect with the product as one of the key ingredients of Chawan junior was Amla which doesn’t go well with milk – a common knowledge which was overlooked by Dabur marketers. Jasravee observes “This is a failure at the product formulation stage and again stems from inadequate understanding of product usage by consumers”. Certain beliefs that are deep rooted in psyche will well serve if respected.

In some cases the companies did not demonstrate adequate understanding of consumer purchase behaviour. An example of this was the launch of pediatric range of Crocin. Crocin syrup containing 125 mg of Paracetamol for 5 ml was introduced for kids to cure cold, cough and light fever. The syrup was introduced directly as an OTC product. Crocin syrup was not very well accepted by the parent community as they did not have the confidence on the product. Jasravee observes that “As with all the Pediatric drugs given to children, the parents always want to take any drugs only after the doctor’s consultation. Even parents who tend to ask chemists for an appropriate medicine of common ailments for themselves will not risk buying anything without the doctor’s advice”. The company had to remedy their strategy and ensure that Crocin Syrup was categorised as a prescription drug by GSK and doctors started providing prescriptions for Crocin Syrup for children.

The study also looks at various other case-lets of launch of Dettol plaster, Dettol shaving cream, and Dettol Extra soap (with moisturiser and a glycerine variant launched as a beauty soap). Sarvajeet observes “I am surprised that they didn’t launch a Dettol toothpaste to fight the germs in the mouth. Imagine using the toothpaste with a pungent taste and a clinical smell early in the morning. Makes you think you have woken up in a hospital bed”.
